When creating your account, we will ask you for the following information:

- Your pet's details (date of birth, weight, species, sex, health conditions, etc.).
- Your delivery address.
- A valid prescription.
You may also provide your membership number for the Mondou Cuddles or Ren's Pets Reward's Club (not mandatory).
